Menggunakan PT :
1. Fotocopy KTP
2. Fotocopy KK
3. Fotocopy Ijazah terakhir
4. Fotocopy Akte Lahir
5. Fotocopy Akte Nikah ( untuk yang sudah menikah )
6. Fotocopy Surat Pengantar dari PT yang bersangkutanTanpa menggunakan PT ( Mandiri ) :
1. Fotocopy KTP
2. Fotocopy KK
3. Fotocopy Ijazah terakhir
4. Fotocopy Akte Lahir
5. Fotocopy Akte Nikah ( untuk yang sudah menikah )
6. Fotocopy Kontrak Kerja yang masih berlakuKhusus untuk tujuan Jepang dan Kore Selatan :
1. Fotocopy KTP
2. Fotocopy KK
3. Fotocopy Ijazah terakhir
4. Fotocopy Akte Lahir
5. Fotocopy Akte Nikah ( untuk yang sudah menikah )
6. Fotocopy Sertifikat Ujian Kelulusan dari LPK / BP2MI
7. Fotocopy bukti kelulusan dari BP2MI

5. Fotocopy Akte Nikah ( untuk yang sudah menikah )
6. Fotocopy Surat Ijin Keluarga
7. Fotocopy Surat Keterangan Sehat
8. Fotocopy Kontrak Kerja dari negara yang bersangkutan
9. Fotocopy Paspor bagi yang sudah pernah keluar negeri
10. Fotocopy Visa bagi yang sudah pernah keluar negeri
11. Fotocopy AK 1 / Kartu KuningMenggunakan PT :
1. Fotocopy KTP
2. Fotocopy KK
3. Fotocopy Ijazah terakhir
4. Fotocopy Akte Lahir
